Partnership for Child Health initiatives have touched the lives of tens of thousands of our community's children and families. By providing integrated services to those with complex medical, mental, behavioral, developmental, and other difficult health conditions, children and youth are now experiencing healthier and more productive lives.

Medical Home

The National Committee for Quality Assurance (NCQA) describes the Patient Centered Medical Home as "a health care setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner."

A Medical Home is...

Children and their families who have medical homes receive the care that is needed from a pediatrician and nurse care coordinator they trust.   • Partnership Support for “Medical Home Improvement” in Pediatric Practices

    The American Academy of Pediatrics (AAP) and the Maternal Child Health Bureau have embraced the Healthy People 2010 goal which states that all children with special health care needs will receive regular ongoing comprehensive care within a medical home. Through its Medical Home Partnership program, the Partnership seeks to expand the medical home knowledge and practice of the community's pediatricians. The Medical Home Partnership program offers education and resources to support primary care providers’ efforts to enhance the delivery of services to children with special health care needs through implementation of the medical home model.
