The Nurse Care Coordination at the Bower Lyman Center for the Medically Complex Child at UF provides care coordination which serves children with complex medical conditions and/or those dependent on medical technology. The Bowman Lyman Center provides a coordinated family-focused and team-based medical home system of care.
All providers collaborate effectively, facilitating communication among primary care physicians, specialists, therapists, schools and providing the continuity of care children with complex medical conditions need to reduce re-hospitalization and promote family well-being and the best possible outcomes for the child.
